An 82-year-old patient presented with a complex Vancouver type B2 periprosthetic femoral fracture, associated with subsidence and instability of the femoral stem.
Given the instability of the femoral component and the fracture pattern, revision of the femoral component was performed using a long cemented Exeter stem.
The periprosthetic fracture was additionally stabilised with a trochanteric plate, providing fixation of the fracture and restoration of femoral stability.
This surgical approach combined revision of the unstable femoral stem with fracture fixation, aiming to achieve stable mechanical reconstruction of the hip.
