A patient presented with a failed intramedullary fixation of a trans-trochanteric fracture (cut-out). The hip screw had been displaced and had become lodged in the acetabulum, causing a bone defect, whilst a pseudarthrosis of the intertrochanteric fracture had developed.

Due to the failure of the initial osteosynthesis and the complex condition of the proximal femur, a revision of the intramedullary nailing was performed as part of a hip hemiarthroplasty.

A modular femoral stem with peripheral support was used for the reconstruction, allowing the prosthesis to be securely anchored in healthier bone. At the same time, the acetabular bone defect was filled with bone grafts.

The aim of this surgical procedure was to restore stability and function to the hip following the failure of the previous osteosynthesis.
